You can notify us of a death using our online form. This can be completed by anyone, but we can only provide facility(s) information and finalise the facility(s) via the Legal Personal Representative (such as the Executor or Administrator).
If this isn’t you, you’ll need to give us the contact details of the Legal Personal Representative when filling out the form. We’ll contact them directly to discuss next steps.

If you are having difficulty obtaining a death certificate, please contact us to discuss alternative documents that may be acceptable.
If we’re missing required information or documents and you’ve provided your phone number, we’ll try to contact you by phone to discuss. We’ll then follow up in writing detailing what we need.
